NBSTSA Privacy Policy Statement

Section 1 - Information we collect:

The NBSTSA collects many kinds of information from multiple sources in order to accurately process applications and to identify and provide you with the best services and experiences.  We value your privacy. 

We collect information when you submit an application either online or by hard copy, contact us via e-mail or phone, when you access our website, and from other related organizations. 

General website statistics are also collected. 

Section 2 – How we use your personal information:

We use certain information such as Social Security Number (SSN), AST membership number, contact information, and your name to identify you in our system. 

Financial information is never shared with any other organizations except those related to processing payment (Wells Fargo Bank and Authorize.Net). 

SSN information is never shared with any individual or organization. 

Contact information such as e-mail address, mailing address, and phone number may be used for authorizing you to test through the 3rd party testing company, sending receipts as requested, general contact, marketing, and general mailing. 

We also collect website access information to give us an idea of how our website is being used, number of page views, etc. This data is usually anonymized and used for statistical purposes only. 

Section 3 – Who we share your information with and what information is shared:

Association of Surgical Technologists (AST) – We share current certification and contact information with AST for the purposes of marketing, statistical data, and validation for AST’s bylaws. AST also shares data with their local state assemblies. 

Association of Surgical Assistants (ASA) – We share current certification and contact information with ASA for the purposes of marketing, statistical data, and validation for ASA’s bylaws. 

Applied Measurement Professionals, Inc. (AMP) – We share your graduation date, school program information, your name, address, telephone number and e-mail, and authorization to test information with AMP for the purposes of allowing you to schedule your examination.  AMP never shares this information with any person or other organization. 

Wells Fargo Bank – We share financial information in order to process your payment.  Check payments are scanned through an online scanning system provided by Wells Fargo and stored in a secured drawer for the period of 30 days until they are shredded by NBSTSA staff.  Credit/debit card numbers are entered into an online payment processing website that is designed and monitored by Wells Fargo.  Receipts that print when payment is completed only include the last four digits of the card number. 

Authorize.Net - You enter your credit/debit card number into the online form to process your payment for applications submitted online.  These numbers are encrypted and only the last four digits are visible to NBSTSA staff. 

NBSTSA, AST and ASA never sell your contact information to others. NBSTSA, AST and ASA always do marketing on behalf of marketers. 

Section 4 – How we protect your information:

Information such as SSN and credit/debit card information is stored in an encrypted format.  Only NBSTSA staff with a username and password may view your SSN. 

If we are unable to successfully charge a credit/debit card number, the application is kept in a locked drawer while we attempt to get in contact with you.  Credit card information is immediately redacted after cards are charged successfully. After a period of time specified by our retention policy, these paper files will be destroyed. 

Electronic application forms will be destroyed after a period of time set by our retention policy.

Section 5 - What information can you opt-out of receiving:

The NBSTSA periodically sends out newsletters, surveys, and special offers by e-mail or mail, and you may opt out of receiving this contact by calling our office or checking the “no” box on an application. 

We share contact information with AST and ASA. Both AST and ASA have their own marketing opt-out system. Please contact them to opt out of their system.
